Hi Baylor,

Thanks for the detailed bug report!  I know whats happening here.  The
MNC internal lookup tables should have a bunch of entries flushed when
the next file in a sequence is created and thats not happening.  I've
added it to the top of my to-do list.

And, in the mean time, your quick fix of increasing MNC_MAX_ID is a
perfectly good work-around.
