Hi Andy,

I just read the exch2 notes and here are a few comments:

 - I don't understand the last sentence in Sec 6.18.4 
   Para 5.  Could you please explain why a tile "would 
   otherwise be excluded"?

 - Sections 6.18.5.x are much improved!

 - I made some small formatting changes--please prefer 
   the "standard" LaTeX constructs (eg. \begin{itemize}...
   \end{itemize}).
