pkg/offline related changes:
- always call MONITOR (monitorFreq has been changed)
- always compile with SOLVE_FOR_PRESSURE call and INTEGR_CONTINUITY call
  (move/remove #ifndef ALLOW_OFFLINE) to allow to compute vertical
  velocity; keep only #ifndef ALLOW_OFFLINE where needed for the adjoint.
