add a new figure, and corresponding text
remove "effective" in many places as this proves to be an ambiguous
term (there are people who call $h$ (thermodynamically) effective
thickness and $hc$ mean (grid cell averaged) thickness, very confusing)
add a few comments, fix a few typos
