 o convert all the '#ifndef "EXCLUDE_MONITOR"'-isms to the now-standard
   '#ifdef "ALLOW_MONITOR"' format as is done with all the other packages
   - note that this also means removal of the monitor package from the 
     packages.conf list for the AD examples
